These Terms & Conditions apply to the use of the services provided by WH Physiotherapy and Sports Performance ("we", "us", "our").
By booking or using our services, you agree to these Terms & Conditions.
1. Our services
WH Physiotherapy and Sports Performance provides physiotherapy, rehabilitation, exercise, taping, cupping therapy and sports performance services.
Our services are intended to assess, manage and support musculoskeletal and physical health, rehabilitation and performance.
The outcome of treatment cannot be guaranteed, as individual responses to treatment and rehabilitation vary.
Where appropriate, we may recommend that you seek assessment or treatment from another healthcare professional.
Our services are not intended to replace emergency medical care. If you are experiencing a medical emergency or symptoms requiring urgent attention, you should contact the appropriate emergency or medical service.
2. Appointments and bookings
Appointments must be booked through the booking method provided by WH Physiotherapy and Sports Performance.
When making a booking, you are responsible for providing accurate and up-to-date information.
An appointment is considered booked once confirmation has been provided.
We reserve the right to refuse or rearrange an appointment where this is necessary for clinical, safety, operational or other legitimate reasons.
3. Arrival and late appointments
Please arrive in sufficient time for your appointment.
If you arrive late, your appointment may need to be shortened so that the next appointment can begin on time. The full appointment fee may still apply.
If you are significantly late, we may need to treat the appointment as a missed appointment.
4. Cancelling or rearranging an appointment
We understand that circumstances can change. If you need to cancel or rearrange an appointment, please provide as much notice as possible.
Appointments cancelled or rearranged with less than [24 HOURS] notice, or appointments that are not attended, may be subject to a cancellation or missed-appointment charge of [AMOUNT / PERCENTAGE].
We may waive this charge where there are exceptional circumstances.
Any cancellation charge will be communicated to you before being applied where reasonably possible.
5. Our cancellation of appointments
We may occasionally need to cancel or rearrange an appointment.
Where we need to do this, we will provide as much notice as reasonably possible and will offer an alternative appointment where appropriate.
We will not be liable for additional costs or losses arising from an appointment that we have had to cancel or rearrange, except where required by law.
6. Fees and payment
The applicable fee for your appointment or service will be communicated to you at the time of booking or before the service is provided.
Payment must be made using the payment method accepted by WH Physiotherapy and Sports Performance.
Unless otherwise agreed, payment is due at the time of the appointment or booking.
We reserve the right to require payment before providing certain services.
7. Treatment and rehabilitation
Physiotherapy and rehabilitation are individualised services. Recommendations will be based on the information available to us and the assessment undertaken.
You should provide accurate and complete information about your health, symptoms, injuries, medication and relevant medical history.
You should inform us if your circumstances or health change during your treatment.
You are responsible for following agreed advice and exercise programmes within your own capabilities and for informing us if an exercise or treatment causes unexpected or significant symptoms.
If you are unsure whether an activity or exercise is appropriate, you should stop and contact us for advice.
8. Health and safety
You should inform us before treatment of any relevant medical condition, injury, medication, allergy or other information that may affect your treatment or participation in exercise.
We may decide that a particular treatment, exercise or service is not appropriate for you and may recommend that you seek further medical assessment.
We may refuse to provide treatment where we believe doing so would present an unacceptable risk to you, our staff or others.
9. Sports performance services
Sports performance services are intended to support physical performance, training and development.
Any performance recommendations, exercise programmes or training guidance will be based on the information available and are not a guarantee of a particular sporting outcome.
You are responsible for using appropriate judgement regarding your own participation in sport and physical activity.
10. Children and young people
Where services are provided to a person under 18, appropriate consent and involvement of a parent or guardian may be required.
We may request that a parent or guardian is present or available during an appointment where appropriate.
11. Clinical records
We maintain appropriate records relating to appointments, assessments, treatment and rehabilitation.
Clinical records will be handled securely and retained in accordance with applicable legal, professional and data protection requirements.

12. Confidentiality
We respect the confidentiality of information provided to us.
Information will only be disclosed to another person or organisation where there is a lawful and appropriate reason to do so, including where necessary for your care, where you have provided appropriate permission, or where disclosure is required or permitted by law.
